1. Why are you joining Kaiser Permanente? We believe this change will allow us to provide our patients with the full range of medical services they deserve.
2. What happens in February 2008? Starting February 1, we will be seeing Kaiser Permanente patients only. Exceptions to this are: - Some PPO patients – check with your PPO plan to see if you can see us as an "out of network" provider
- Patients who choose to pay cash at the time of service
3. Will all eight doctors and staff remain at the same Boscell Road location? Yes, and you will be able to continue seeing your own doctor if you fit within one of the categories in item 2. 4. If I choose to keep my current insurance and do not fit within any of the categories in item 2 where do I take my children after February 1st? 5. If I choose to stay with my primary pediatrician beginning February 1st, what do I need to do? You would have to (1) enroll in a Kaiser Permanente health plan or (2) fit within one of the categories in item 2. Information on Kaiser Permanente is available at: www.kp.org. 6. If I choose to change MDs, how do I transfer my medical records? Allow 5 business days to process your medical records transfer request. To request for medical records to be transferred out or into the office, click on "Patient Forms" then click on "Authorization For Release of Medical Records". Print and complete the form. Forward your request to the attention of Medical Records Department. The form can be faxed to our office at (510) 979-0798. |
