Dear United Health/Pacific Care families,

You recently received a letter from us about our affiliation with Kaiser Permanente and termination of our contract with United Health Care as of November 17, 2008. UHC has decided to change that date and now our contract with United Health Care will be terminating January 15, 2008. If you have not already received a letter from United Health Care you should receive one shortly about selecting another UHC contracting pediatrician.

We understand that choosing your family's health insurance is an important decision. Most of you have a choice of health plans and Kaiser Permanente may be one of your options. You may talk to your doctor about any questions you have. We appreciate the privilege of being part of your lives. For those of you who choose to remain in our care after January 15, we look forward to continuing this relationship for many years.
